  • Submit an Incident Report (iReport) via WSH Incident Reporting

    Employers or occupiers can submit a work-related iReport. Injured employees or their dependents can notify MOM of work-related accident or Occupational Disease. You can also amend, purchase or download iReport.

  • MOM - Amendment/Deletion of Incident Report (iReport)

    Estimated time to complete: 5 mins

    You are able to request amendments or deletions to your iReport submissions, particularly for fields that cannot be modified directly in the WSH Incident Reporting eService (those appearing as greyed-out) due to cases being in processing or follow-up stages.
