This form is for employers whose workers are occupiers at HDB addresses and are unable to register the workers after:
- Property owner has registered the workers as occupiers with HDB
- The workers’ FIN number has been updated with HDB
This form is for OFWAS/EP eService users who encounter an error message that the address is not approved for residential use, and the employer has relevant documents to show that the premise is approved for residential use by the respective agencies.

- Update of the correct occupancy load of the premise
- Removal of pass holders no longer staying at the address
This form is for OFWAS/EP eService users who encounter an error message that the postal code, floor or unit number is invalid although it is a valid address.
This form is for property owners who require assistance to remove ex-tenants from their address as they are unable to access MOM’s Foreign Worker Tenant Enquiry Service (FWTES) to remove their ex-tenants/check on the tenants registered at their property. (Property agents may also submit on their behalf.)
