Inquiry Committee into the Combustible Dust Explosion at 32E Tuas Avenue 11 Begins Work

  1. Minister for Manpower, Mrs Josephine Teo, has appointed an Inquiry Committee under the Workplace Safety and Health (WSH) Act to inquire into the causes and circumstances that led to a fatal fire and explosion at 32E Tuas Avenue 11 on 24 February 2021.  
  2. Senior District Judge, Mr Ong Hian Sun, will chair the Inquiry Committee. He will be assisted by two assessors, Er Lucas Ng Hong Kiang, General Manager of Plant, Petrochemical Corporation of Singapore (Private) Limited, and Dr Peter Nagler, Chief Innovation Officer at A*STAR.   3. The Inquiry Committee will look into and ascertain the causes and circumstances that led to the explosion and make recommendations to prevent the recurrence of such accidents. The full Terms of Reference ("TOR") for the Inquiry Committee can be found in Annex B. The date and venue of the Inquiry will be announced at a later date. 
  4. The Inquiry Committee began its work on 5 March 2021. They visited the incident site with Senior Minister of State for Manpower, Mr Zaqy Mohamad, and were briefed on the status of investigations.
