Application for Designated Workplace Doctor

You may apply to be a Designated Workplace Doctor online.

Application Criteria

• Applicant must be at least 21 years old.
• Applicant must be a registered medical practitioner with the Singapore Medical Council.

Required Documents

• Copy of NRIC/Work Pass (both sides).
• Copy of Basic Medical Qualification which complies with the requirements of the Medical Registration Act (Cap 174) e.g. MBBS - Bachelor of Medicine and Bachelor of Surgery.
• Copy of Valid Medical Registration Certificate (Full Registration or Conditional Registration) issued by Singapore Medical Council.
• Copy of Valid Practising Certificate (Full Registration or Conditional Registration) issued by Singapore Medical Council.

For All Hazards

• Copy of approved Postgraduate Occupational Health Qualification e.g. M Med (OM), Graduate Diploma in Occupational Medicine (GDOM).

For Compressed Air Environment Hazard Only

• Copy of Hyperbaric or Underwater Medicine qualification e.g. NUMOC (Naval and Undersea Medical Officer Course)

Important Points

• All details must be entered in BLOCK letters.
• All documents must be submitted in Portable Document Format (PDF) only.
• Application with incomplete information and insufficient supporting documents will be rejected.

Processing Time

Application will be processed within 8 weeks, upon receipt of payment and supporting documents.
