The following are the documents required for the various medical monitoring reports:
Summary report of examinations:
- SingPass
- Workplace number of the company you are submitting the summary report for
- Completed summary report of examinations
- Name of designated factory doctor who conducted the medical examinations
- For noise hazard, results of abnormal audiograms for noise hazard
- For other hazards, copies of all the test results
Register of workers:
- SingPass
- Workplace number of the company you are submitting the register of workers for
- Completed register of workers
- Name of designated factory doctor who conducted the medical examinations
- Copies of test results
Certificate of suspension:
- SingPass
- Workplace number of the company where the affected worker is employed
- Details of the affected worker including name, NRIC or FIN, occupation and department
- Copies of test results
Certificate of fitness:
- SingPass
- Workplace number of the company where the affected worker is employed
- Details of the affected worker including name, NRIC or FIN, occupation and department
- Copies of test results
